Create User-Friendly Forms in Request Tracker with FormTools
The free FormTools extension for RT allows you to easily create custom, multi-page form wizards to help your users open new requests. You can build pages to collect information about the new request, which will all be automatically stored in custom fields when the new ticket is created. You can also add custom sections to give users additional instructions for opening requests.

Privileged Users
Forms can be shown to privileged users on your system, making it easier to create new tickets. Even if your users can currently create tickets directly in RT, a clean, simple form can make it easier by showing only the fields they need to fill out.
Self Service
If you grant self service access to unprivileged users, they can also access forms. Guiding users to forms allows you to provide additional help text right in the form to help them fill out the correct information. And users no longer need to think about which queue to select because the form will always create a ticket in the correct queue.
